San Jose Unified School District provides medical, dental and life insurance benefits to employees, retirees, and their eligible dependents.
Most benefits coverage begins on the first of the month following the completion of your employment waiting period. In order to determine your benefits eligibility status (active full-time employee, part-time employee, early retiree, retiree age 65+, leaves of absence, or other bargaining unit arrangements) please contact the Human Resources Benefits department. Part-time employees regularly scheduled to work at least 20 hours per week are eligible for benefits; seasonal and temporary employees are not eligible.
All benefits-eligible employees must complete an enrollment form, regardless of whether you are required to make contributions towards your coverage elections. All enrollment forms must be submitted to the Human Resources Benefits department prior to your effective date of coverage.
You may enroll yourself and your eligible dependents for medical and dental coverage. Your eligible dependents generally include:
Employees have a choice of medical and dental plans. The employee and eligible dependents must enroll in the same plans.
COBRA BENEFITS (CONSOLIDATED OMNIBUS RECONCILIATION ACT OF 1985)
Federal law requires most employers sponsoring group health plans to offer employees and their families the opportunity for a temporary extension of health coverage (called “Continuation Coverage”), at group rates in certain instances where coverage under the plan would otherwise end, (e.g., dependent children beyond age 19 or full-time students at age 25/24 if enrolled in Kaiser). Employees and their dependents should read the COBRA section carefully.
